Referring Yourself

If you are interested in referring yourself to any of our programs please be sure to call our office at (765)659-4739 or toll free at 1-866-80-RAINBOW. A staff member will be glad to speak to you to help you determine whether our services would be a good fit for your needs. If you are interested in referring yourself to the Residential Program the process usually takes about a week from the first phone call to placement in our home if we have openings. Referring a Client

If you are interested in referring one of your clients to our Mandatory Program please call our office to speak with the Executive Director.

Resident Application Packet

If you think that Rainbow Haven looks like the place for you please take a minute to look over our application packet. Print off the following documents, fill them out, and get them back to us as soon as possible to be considered for our next opening. Completed application packets may be mailed, faxed, or hand-delivered to our office.
